Forms
The Foundation requires grantees to submit both periodic
program and financial reports.
• Foundation staff will determine whether grantees should
provide program reports once a year or twice a year.
• Financial reports on grant expenditures must be submitted
twice a year.
Additional forms grantees might need cover budget revisions and
grant extensions.
• Budget revisions must be submitted to and approved by the
Foundation prior to funds being allocated to different line items.
• If a grantee feels they will need extra time to achieve
program objectives, they can request a grant extension. These
are submitted six months out from the end of the grant
period.
